Apicoectomy is an oral-surgery procedure that removes the top of the root of the tooth(apex), mostly due to chronic inflammation or fracture at the top of the root (granuloma or cyst). Apicoectomy is performed under local anesthesia, which means that the treatment is painless.

HOW DOES THE PROCEDURE GO?

At the beginning of the procedure, the gum is cut and lifted to reach the root of the tooth. Then, the top of the root with the inflammatory process is removed, and the root canal is filled with a variety of materials and mixtures which are intended for it. Depending on the individual case of the patient, the tooth can be treated endodontically (cleaning and widening of the tooth canal, and its filling) prior to surgery or during the surgery.

If you feel pain or swelling in the tooth that has been endodontically treated, it is necessary to seek the help of a dentist. In this case, the dentist will do X-rays of that area (of the teeth and the surrounding bone) and a clinical examination to decide whether to make a picoectomy. Root canals are very complex, especially around the top of the root. Infectious debris is sometimes left behind in the branches of canal after an endodontic treatment. This can lead to re-infection and a later need for apicoectomy. In this procedure, the dentist removes the top root together with the inflamed and infected tissue, and seals the top of the root with the filling of canals. Apicoectomy is now becoming an increasingly common procedure, because it allows the preservation of patients’ teeth that cannot be cured with endodontic therapy and which would normally have to be removed. It is considered a small surgical procedure, and takes 30-60 minutes, depending on the tooth region. Procedures on the front teeth are generally shorter, while those on the lower molars are the longest.

1. After surgery, it is necessary to put a cold compress on the procedure location to reduce swelling
2. Taking painkillers recommended by dentists in the case of pain
3. Taking antibiotics to shorten the postoperative recovery
4. After 7 days, stitches are removed, and during all this time it is necessary to maintain maximum oral hygiene and brush around the wound with gentle movements to remove plaque
5. Gargling with sage tea, salt water, or an antiseptic
6. In the case of mild bleeding from the stitches, a damp bag of tea can be applied on them (black tea is recommended)
7. After the procedure, it is preferable to eat liquid food, hot drinks and hot food should be avoided
8. It is recommended to avoid smoking immediately after the procedure
9. It is recommended to avoid alcohol consumption
